The following must be produce during the screening exercise in Nation Diploma program in FEDPONEK

2. Receipt of payment fees

3. Certificate of medical fitness

4.Originals and photocopies of the following:

a. Certificate(Academic)

b. Birth certificate/Declaration of age

c.Letter of Attestation/Testimonial

d.Original Jamb slip( for ND regular students) ORIGINALJAMBSLIP

e. Two (2) recent passport photographs certified at the black

BELOW ARE THE COMPLETE COURSE REGISTRATION FORMS COMPRISING:

1. Course Registration form (2 copies) duplicate

2. Course Semester forms( triplicate)

3. Collection of anti-cult form

NOTE: (1),(2),(3) should be completed in the various departments below:

a. HOD to sign the appropriate areas

b. HOD to forward completed and signed forms to Admissions/Records and Statistics Departments.
